Submitting the order form below is the first step for all new Sparklist clients; we use it to start your Sparkcampaign account set-up and to keep a record of your preferences.

After completing this form, remember to send us a signed Service Agreement.

If you plan to create multiple lists with unique administrators, you can submit their information in the "Instructions" field at the end of this form. Once your account is configured, you can also change or add to this information at any time.

List Configuration
List Name* Each list that Sparklist hosts has a "name" (e.g., yourlistname@). Please read the following important guidelines for selecting a list name. Then, enter your preferred list name in the field below. Select your list name carefully—once configured, it cannot be changed! Make your list name descriptive (e.g., dailynews@, sales-specials@, etc.). Consider the context of any custom domains that you may use (e.g., @lists.yourdomain.com). Try to limit your list name to fewer than 15 characters, to prevent it from word-wrapping. Use a hyphen (-) or an underscore (_) if your want to separate multiple words (e.g., music-talk@). Blank spaces are not allowed! If you want us to set up multiple lists for your account, use the "Instructions" field at the end of this form to indicate the names of your other lists. Please note that all set-up fees are non-refundable, and that we require a six (6) month minimum term of service. Credit Card Information
You may submit your credit card information by this secure online form, fax, or telephone. (Regardless of which payment method you use, please submit this form so that we may process your order correctly.) We retain your card information in a secure file, and automatically charge it with your invoice amount each month. We then send you a monthly billing statement by email of your actual usage and total charges. Please remember that we bill your account for your selected package's minimum monthly fee even if you have no messaging activity during the month. Also remember that we require a six (6) month minimum term of service for all new and upgrade accounts. Payment by Purchase Order
Payment by purchase order is only available for accounts over $500 per month, from organizations located in the United States or Canada. If you would like to pay by pre-approved purchase order, please fax it to us at (510) 844-1598. Your first invoice will include your package's monthly service fee and the non-refundable set-up fee. Please note that your purchase order must allow us to bill you automatically on a monthly basis. We will send you a monthly billing statement by email of your actual usage and total charges for each month. Payment is due within 30 days of the invoice date, and we require a six (6) month minimum term of service.
